ABSTRACT

The 24th National Conference of Neurology of Chinese Medical Association was held in Zhuhai City, Guangdong Province during September 23-26,2021.The conference adopted a combination of online and offline methods, with a total of 2 plenary meetings, 20 special seminars, 284 invited reports, 382 papers exchanged at the conference and 1 088 papers exchanged on the wall. The conference focused on cerebrovascular diseases, epilepsy, cognitive disorders, myopathy, peripheral neuropathy, neurodegenerative diseases, nerve infectious diseases, demyelinating diseases, neuroimmune diseases, genetic and metabolic nerve diseases, nerve rehabilitation, anxiety and depression, headache, sleep disorders, nerve nursing, nerve intervention, neuroimaging, neuroelectrophysiology, translational medicine, precision medicine and other related nervous system diseases. There were more than 7 000 participants who attended this conferece.

ABSTRACT

@#With the increasing popularity of chest spiral CT screening, the detection rate of lung cancer in China is increasing. According to the characteristics of lung cancer in China and the progress of lung cancer researches at home and abroad, Chinese Medical Association guidelines for clinical diagnosis and treatment of lung cancer are updated once a year. It is of great guiding significance to standardize and improve the clinical diagnosis and treatment of lung cancer for thoracic surgeons. The surgical diagnosis and treatment of lung cancer in the guidelines mainly include: (1) surgical treatment of stage Ⅰ-Ⅱ non-small cell lung cancer (NSCLC); (2) surgical treatment of resectable stage Ⅲ NSCLC; (3) surgical treatment of multiple primary lung cancer; and (4) surgical treatment of locally resectable small cell lung cancer. Based on the Chinese Medical Association guidelines for clinical diagnosis and treatment of lung cancer (2019 edition), this paper interprets the hot issues related to the surgical treatment of lung cancer.

ABSTRACT

Objective@#To explore the effect of family doctor team service in the medical association model on empowerment behavior and health status of elderly patients with diabetes in the community.@*Methods@#A total of 158 patients with diabetes who were diagnosed with free physical examination and were diagnosed as diabetic patients in 4 communities under the jurisdiction of Fuzhong Community Health Service Center From March 2018 to May 2018, we conveniently divided into intervention group (79 cases) and the control group (79 cases), according to whether or not to sign a family doctor. The control group was older people who did not sign up for a family doctor, the control group was given routine intervention according to the community chronic disease follow-up mode, while the intervention group gave the contracted family doctor on the basis of routine intervention, and carried out individual assessment, group management, group intervention, one-on-one guidance, and the body mass index, abdominal circumference, fasting blood glucose, HbA1c, self-management ability of the two groups were evaluated before intervention and 6 months after intervenation.@*Results@#There was no significant difference between the two groups before intervention (P >0.05). After 6 months of intervention,abdominal circumference, fasting blood glucose and HbA1c was (79.5±5.0) cm, (6.1±0.7) mmol/L, (6.3±0.6)% in the intervention group, (85.2±11.4) cm, (6.5±0.8) mmol/L, (6.8±0.7)% in the control group, and there was significant difference between the two groups (t=2.142, 3.345, 4.820, P<0.05 or 0.01); the self-empowerment ability and the self-management was (37.7±1.7), (44.3±6.8) in the intervention group, (30.1±2.5), (36.5±2.8) in the control group, and there was significant difference between the two groups (t=22.344, 9.427, P<0.01) .@*Conclusion@#The family doctor team service under the model of medical association can improve empowerment behavior of elderly diabetic patients in the community and improve their health status.

ABSTRACT

Under the background of the innovative medical association, the company took the lead in promoting the senior nurses to the community hospitals in Anhui, and opened community nursing clinics to better implement the graded diagnosis and treatment system and meet the needs of residents' integrated health services. From the background of a new round of medical reform, this paper analyzes the new occupational orientation of senior nurses in the innovative medical association model, and introduces and analyzes the problems that arise in the exploration stage of the sinking community, and puts forward suggestions based on the actual situation.

ABSTRACT

In this article, I looked at the life of Yun Il-sun, a representative medical scientist of modern Korea, and examined the following problems. First, I took note of the position of the Korean people in the academic system of the Japanese colonial empire and restored the life of Yun Il-sun as specifically as possible. Yun was educated among Japanese people from elementary school to university. Although he received the best education at Old System High School and Imperial University and grew to be a prominent medical scientist, he could not overcome his identity as a colonized. Yun Il-sun, who moved from Keijo Imperial University to Severance Union Medical College, involved in activities founding of the Korean Medical Association and the Korean Medical Journal. Second, I the meaning of ‘culture’ to the intellectuals in the periphery. Old System High School and Imperial University where Yun Il-sun was educated were the hotbed of ‘culturalism.’ Yun's college days were the heyday of Taisho Democracy, and students were attracted to Marxism, Christian poverty movement, Buddhist cultivation movement and so on. Yun sought to overcome the ideological of young people through the acquisition of ‘culture.’ The ‘culture’ emphasized by Yun had an enlightenment characteristic that emphasized education, but it also functioned as a‘identity culture of educated elites.’ Third, I used the concept of ‘colonial academism’ and examined the aspects and characteristics of the colonial-periphery academic field, focusing on medicine. Yun Il-sun was a Korean professor at the Keijo Imperial University. He founded an academic society and published an academic journal for Koreans. He attempted to reproduce scholarship by doctoral dissertations. At the same time, several facts show that he was also in the affected area of ‘colonial academism’: the fact that he was kicked out of the Keijo Imperial University, the fact that the Korean Medical Association and the Korean Medical Journal were banned by Governor General, the fact that his students asked for doctoral degrees from Kyoto Imperial University where he studied. Yun Il-sun crossed the limits of ‘colonial academism’ and acted as the agent of empire. This was made possible by the characteristics of the academic discipline of medicine, the environment of the Severance Union Medical College, and personal traits of superior ability and indifference to politics. I the postcolonial evolution of the ‘colonial academism’ and ‘culturalism.’ The mix of continuity and discontinuity from ‘colonial academism’ and the hybrid of Japanese academism and American academism, the Korean characteristics of ‘postcolonial academism.’ Yun tried to harmonize the American academism with the Japanese academism and the purity of academism. This effort was revealed as an emphasis on basic medicine and natural sciences. As combined with culturalism and indifference to politics, he was recognized as the symbol of ivory tower and academism.

ABSTRACT

In June 27, 2017, there were two acupuncture clinical trials published in (). The conclusions were that electroacupuncture could relieve the leakage of urine of female stress urinary incontinence and that acupuncture with clomiphene or not could not improve the live birth rate of female with polycystic ovarian syndrome, compared with shallow insertion at non-acupoint combined with clomiphene placebo. The Youth Committee of the China Association of Acupuncture-Moxibustion held an academic salon about the above two trials on September 7, 2017. It is considered that rigorous design is key to clinical acupuncture-moxibustion trial, that acupuncture-moxibustion therapy is with psychosomatic medicine theory. The clinical acupuncture-moxibustion research method was discussed, including study group selection, acupuncture control, the primary outcome, the interpretation of result, etc.

Objective How to make good use of the construction opportunity of the medical association to integrate resources,build scientific research platform and establish practical operation mechanism,to improve the technology innovation ability of district county-level hospitals.Methods Through literature analysis and empirical study,this paper discusses the feasibility and operation mechanism of the construction of research platform in district county-level hospitals.Results The research platform can be built with the help of medical association construction.The operation mechanism can effectively improve the research level of district county-level hospitals,by taking the chance of discipline construction,implementing the institutional mechanism for rewards and punishment.Conclusions With the help of superior medical experts,two-way referral,academic lectures and other activities,advanced scientific culture idea was spread;The training of scientific research personnel,resource integration,and project cooperation increased opportunities.All of this to set up a new platform for district county-level hospitals to promote their scientific and technological innovation abilities.By taking the chance of discipline construction,clarified target responsibility,to carry out the rewards and punishment system and the implementation of the scientific and technological innovation incentive mechanism,can force them to actively promote the ability of scientific and technological innovation,actively carry out scientific research and improve the level of business.

ABSTRACT

Objective: The study aimed to explore the initial effect of the Luohu healthcare group reform in Shenzhen based on the patient’s perspective. Methods: The questionnaire survey was conducted to investigate the views of patients in 3 hospitals and 23 community health centers in Luohu district. Results: A total of 936 valid ques-tionnaires were collected and analyzed. The valid response rate was 94. 55% . More than 90 percent of the total sur-veyed patients provided a good or very good evaluation on the healthcare services, service attitude, medical referral services and interpretation, communication skills, and better family doctor skills. More than 80 percent of the sur-veyed patients perceived well or very well on medical treatment, the convenience of medical referral, the referral process of the community health centers, and the specialists serving in community health centers mechanism. Conclu-sions: The results showed that patients had a slightly high satisfaction with family doctors and community health cen-ter, and after the group was founded, the proportion of patients who preferred going to community health centers after illness has been increasing significantly more than before. In future, a further development of the advantages of healthcare group should be brought into action and improve the relevant supporting policies. The long-term effect de-serves a further observation.

ABSTRACT

Objective: The study aimed to explore the initial effect of the Luohu Healthcare Group Reform in Shenzhen based on the employee’s perspective. Methods: The questionnaire survey was conducted to investigate the views of employees in 3 hospitals and 23 community health centers in Luohu district. Results: A total of 480 ques-tionnaires were distributed and only 412 valid questionnaires were collected and analyzed. The valid response rate was 85. 83% . 37. 21% of doctors responded that they had motive power to work in community health centers; 75. 38 per-cent of employees stated that their healthcare organization changed a lot; Employees’average work satisfaction scores, interpersonal relationship and healthcare organization development were all above 4. 20, and the consistency of em-ployee evaluation was high. Conclusions: Most of the surveyed employees evaluated the initial stage of the Luohu health reform positively. Since the hospital group started to operate two years ago, the development of the affiliated health facilities has been on an upward trend. The formation of vertical alliance within the hospital group has e-merged; and physicians of secondary and higher-grade hospitals have had increasing willingness of working at the community health centers, especially in the departments of internal medicine and traditional Chinese medicine. The hospital group has been guiding all affiliated facilities to develop its unique signature specialty so that facilities can col-laborate with each other and evolve as a whole. Group employees have expectations that the hospital group will coordi-nate and incorporate interests of all stakeholders, fulfill the commitments, and develop a better future for the group.

ABSTRACT

The Korean Medical Association (KMA) has been requested to respond proactively and effectively to local agendas reflecting global perspectives in order to lead our society through the dynamic and fast-changing health care environment, which faces issues such as big data, artificial intelligence, and the increasing health-related impacts of climate change and fine particulate matter. Since the KMA joined the World Medical Association (WMA) in 1949 and the Confederation of Medical Associations in Asia and Oceania (CMAAO) in 1956, the KMA has made continuous progress in international affairs. Based on these achievements, the KMA has recently played important roles, such as the chair of the WMA Finance and Planning Committee, a council member in the WMA, and a council chair in the CMAAO. For the next several decades, the WMA and CMAAO are still expected to be the basic channels for sharing trends and visions, discussing health challenges across borders, and developing powerful initiatives that improve people's health. Maintaining current partnerships with the WMA and CMAAO and strengthening strategic collaboration with promising national medical associations are required to extend the KMA's influence. Training next-generation representatives to build capacity for international relations might also be necessary for the sustainability of international collaboration. Through the KMA's advancements in international affairs, incorporating harmonious perspectives on local, regional, and global agendas, the KMA is fully expected to contribute to addressing health challenges by demonstrating its leadership.

Además se dará una explicación breve sobre el uso de tabla de enfermedad coronaria para la asignación de porcentaje basado en las Guías de Valoración del Deterioro Permanente de la Academia Americana de Medicina y con ello brindarle al médico forense las herramientas necesarias para asignar de forma objetiva un porcentaje de pérdida de la capacidad funcional por los efectos de la cardiopatía de origen isquémico.


AbstractIn this article will be described the necessary basic elements to be taken into account when using a forensic medical approach to diagnose a person with ischemic heart disease, both in clinical history, physical examination and complementary studies. In addition, is a brief explanation of how to use the table of coronary heart disease for the allocation of a percentage based on the Guidelines for Evaluation of Permanent Impairment of the American Academy of Medicine and thereby provide to the medical examiner the necessary tools to assign subjectively the percentage of loss of functional capacity by the effects of ischemic heart disease.

Objective:To investigate the therapeutic effect of stroke patients received rehabilitation treatment at different medical units in the Rehabilitation Medical Association.Method:Totally 183 stroke patients were recruited from October 12,2012 to February 5,2016 by Shenzhen Rehabilitation Medical Association(SRMA).SRMA consists of 4 members,including the Second People's Hospital of Shenzhen,Shenzhen Nan'ao People's Hospital,Shenzhen Baoxing Hospital and Community Healthcare Center in Futian District,which belong to tertiary hospital,secondary hospital,and primary healthcare unit,respectively.According to the time of onset of stroke,all the patients were divided into recovery phase or sequelae phase.The rehabilitation facilities were standardized.All patients received rehabilitation treatment for 4 weeks,which included functional rehabilitation training of limb,functional electrical stimulation training,activities of daily living training.Fugl-Meyer Assessment(FMA),balance subscale of the Fugl-Meyer test(FMB),National Institute of Health Stroke Scale(NIHSS) and modified Barthel Index(MBI) before and after rehabilitation treatment were used to evaluate the effects.Result:The scores of FMA,FMB,NIHSS and MBI were significantly improved in all the groups after rehabilitation treatment(P < 0.05).There was no significant difference in all assessments among the groups(P> 0.05).Conclusion:Based on the standardized rehabilitation treatment programs,the stroke patients come from the members of Rehabilitation Medical Association can achieve homogeneous effects after rehabilitation treatment.

Starting from the experiment of Zhenjiang Rehabilitation Group in establishing a tightly-organized medical association,the authors detailed the practices and outcomes of the group in deepening the system and mechanism reforms,promoting the hierarchical medical system,and other reforms of public hospitals.Key roadblocks and causes were identified,with reform measures proposed as well.
